A useful decision leaves evidence that can be checked.

A decision is more than a task title. It needs context, a constraint, a consequence and a reason to reopen it later.

NOTE / 01

Record the pressure

Timing, risk, dependencies, available data and user rights determine a reasonable choice. Without context, the record looks like a universal rule.

NOTE / 02

Name the rejected option

One short sentence about an alternative shows what was traded away. This prevents the same question being solved again without new facts.

NOTE / 03

Connect consequence to verification

If a decision promises less input, a faster response or simpler handoff, name the check that will show whether it happened.

NOTE / 04

Define the return signal

A new data volume, changed legal duty or different failure pattern can provide a clear reason to revisit the choice.
